a. Confidentiality
All information you give me will be kept in confidence. Your personal details and medical information will not be made available to third parties unless permission to do so is granted by you.
b. Recommendations
Using my judgment and experience, I will suggest certain exercises and other lifestyle advice that I believe will help you achieve your personal goals, but if at any time you have specific requests please tell me so I can accommodate them.
c. Referral
I intend to work with you within the scope of my knowledge and competencies as a REPs Registered Exercise Professional. Therefore, when I believe it is in your best interests to see another health professional, I will refer you appropriately.
d. If I (the trainer) cancels or runs late
Unforeseen circumstances may arise which require our sessions to be rescheduled. In this event the following policy will apply:

a. Disclosure of information
Please disclose all health information as requested at our initial consultation and keep me updated and informed of any changes to your health status. This includes all medical conditions; physical and mental, injuries, allergies, and medication you are taking. If necessary, you may need to seek clearance from your doctor before participation in the exercise programme I recommend.
b. Accepting potential risks
Whilst for most people exercise is hugely positive with many health benefits, there remains some risk that I require you to be aware of and for you to sign the informed consent and disclaimer that I will provide.
You must also complete a Physical Activity Readiness Questionnaire (PARQ) prior to engaging in any physical activity prescribed by the trainer. Answers to the PARQ must be accurate to the best of your knowledge, and you understand that it is your responsibility to inform the trainer of any changes.
c. If you (the client) cancel or run late
If you cancel your session with less than 24 hours’ notice or fail to show you will be required to pay the full amount of the session fee. If you have purchased multiple sessions, then one session will be deducted from your package.
If you cancel your session with more than 24 hours’ notice, you will not forfeit a session, or be liable to pay for the session, with the following exception:
If you (the client) are late to a session, I will do my best to get the most out of the remaining time left in the session as I am unable to run over time due to previously arranged client commitments.
The above terms outlined in section 2.c only apply to face to face personal training, not online coaching.

e. Session options and terms
Face to face personal training and hybrid training session length will be the agreed length as outlined in your package, as will session frequency. If you have a hybrid program, the agreed amount of workouts per week will also be provided through the DeadFit app.
Online coaching workouts will be reviewed on a case by case basis, and the price of the program will not be determined by the number of sessions each week.
This agreement will run for an initial period agreed by the client and trainer, and will be outlined in your package.
Once the initial term has been completed, the agreement will run until further notice (please note that the agreement cannot be cancelled during the initial term). Any request to cancel this agreement after the initial term is completed is required to be in writing with 30 days notice i.e. email or message.
If you have purchased a number of sessions, these will only be valid for 45 days from the date of purchase unless an agreement has been made between the trainer and client.
f. Payment option
This program has been designed for the client and tailored specifically for their goals. The minimum term of this agreement, and it's monthly price, is outlined in the package purchased. After this term, payments will continue until the client gives 30 days notice to cancel.
If you have purchased a session package, the terms will end when the sessions have been used. After the sessions have been used, more will need to be purchased in order to continue training with the trainer.
Payments are non-refundable and the client assumes responsibility for paying the trainer in a timely manner. The following exceptions apply:

While I can give you the tools and encouragement to reach your goals, ultimately you will need to assume responsibility for making the required changes. I can best help you with this process if you keep me fully informed of any issues that arise for you and if you have any questions or concerns whatsoever please don’t hesitate to voice them with me.
The client understands that they are paying the trainer for their services as outlined in this agreement. In the event that the client doesn’t use the services, the client will still be responsible to make payment for the full initial term as agreed above. The trainer will deliver their sessions and advice to the best of their ability, will be registered with REPs, and will respect all applicable UK laws and the REPs Code of Conduct at all times.
The client agrees to make all payments by the due date.
Refunds and cancellations can be discussed on an individual basis. If a refund or cancellation is agreed, the below will apply:
The results of any training program are dependent on your effort and co-operation both in and outside of the sessions. No particular result is guaranteed by the trainer.
ACKNOWLEDGEMENT OF THIS AGREEMENT - I have read and understood this agreement.
The purpose of an exercise program is to help you to achieve health and fitness goals. You will be set a program based upon your present activity/exercise levels and your stated goals. You will experience some feelings of exertion during each activity session and may become hot and uncomfortable at times. If your plan includes certain types of cardiovascular exercise you can expect your breathing to become more rapid and your heart rate to increase.
As your fitness improves, your goals may lead you to participate in more vigorous levels of activity but these should remain within your capabilities. All activities will be explained and demonstrated to you (either in person or through the GDT app), but you should feel free to ask any questions you may have. Any exercise program carries with it an element of risk.
The sessions are designed to minimise risk yet, at the same time, provide an effective exercise/activity program. Please inform the instructor if there is any reason why you should not participate in an activity i.e. if you have an illness or an injury which might be aggravated by exercise. If, at any time, you feel undue pain or excessive discomfort stop the activity immediately and inform the instructor of your symptoms. You are free to withdraw from any activity at any time you wish.
If you are performing workouts at home, please ensure that the area you are working is safe and free from obstructions. make sure there is enough floor space, and that all tables and chairs have been moved. Also check for high level objects such as lights and cabinates.
By agreeing to this, you agree to take part in the program described to you by the instructor. The nature, purpose, risks and benefits have been explained to you and you understand what is required of you, and that you may withdraw from physical activity at any time.
